the U.S. Principal Party-in-Interest (“USPPI”), organized and doing business under the laws of the United States in the State of

hereby authorizes UPS Worldwide Forwarding, Inc. and its affiliates (“Forwarding Agent”), its successors or assigns, to act for and on its behalf as a true and lawful agent and attorney of the USPPI, from this date, in the United States either in writing,
electronically, or by other authorized means to:

Act as Forwarding Agent for export control, Bureau of the Census reporting, and customs purposes, and to make, endorse, sign or file Electronic Export Information (“EEI”) through the Automated Export System (“AES”) or to perform any act which may be required by law in connection with the exportation or transportation of any merchandise shipped or consigned by or to the USPPI and to receive or ship any merchandise on behalf of the USPPI.

The USPPI hereby certifies that all statements and information contained in the documentation provided to the Forwarding Agent relating to the exportation or transportation are true and correct.
Furthermore, the USPPI acknowledges that it is responsible for determining export licensing requirements and obtaining licensing authority.
The USPPI understands that civil and criminal penalties may be imposed for making false or fraudulent statements or for the violation of any United States laws or regulations on exportation.

The USPPI agrees that by executing this document, it is subject to the most current version of the UPS Tariff/Terms and Conditions of Service, which are published electronically on the UPS website (ups.com®).
The most current version of the UPS Tariff/Terms and Conditions of Service shall govern the shipment of packages tendered to the UPS Forwarding Agent.

The USPPI agrees that the most current version of the UPS Tariff/Terms and Conditions of Service in effect at the time USPPI tenders a shipment for the use of Forwarding Agent’s services will apply.
If the USPPI tenders the shipment for export using the Forwarding Agent’s services through a UPS automated shipping system such as UPS WorldShip™, then the shipment shall be subject to the terms and conditions of the UPS Technology Agreement that grants a limited license to use the UPS technology to aid in the transportation of shipments by the USPPI and Forwarding Agent.

The USPPI hereby certifies that it has obtained all required licenses or other authorizations necessary to ship any commodity listed on the documentation provided to the Forwarding Agent.

The USPPI further certifies that all statements and information pertaining to exportation contained in the documentation provided to UPS are true and correct.
The USPPI agrees to provide the Forwarding Agent with correct and complete information regarding points of contact, addresses and telephone numbers, quantities, weights, values and any other information required for exportation.
UPS will not disclose any confidential information to third parties except in compliance with U.S. law.
UPS agrees to maintain the confidentiality of any information that may be required for export.
